The Center for Disease Control (CDC) offers up-to-date information regarding flu information, patterns, trends and considerations.

Flu Vaccine Details

Decatur County Memorial Hospital offers flu vaccine clinics throughout the local area and is recommended to print and complete the consent form below to speed the process.  Vaccines are free of charge by virtue of major support from the Hospital Foundation of Decatur County.  The chart below lists the dates and times of the scheduled clinics:

With assistance from the Hospital Foundation of Decatur County, DCMH provided nearly 3,000 flu vaccines to the community in the Fall of 2012 to help minimize the impact to the community.
