Flu Vaccine Details
Decatur County Memorial Hospital offers flu vaccine clinics throughout the local area and is recommneded to print and complete the consent form below to speed the process. Vaccines are free of charge by virtue of major support from the Hospital Foundation of Decatur County and additional support from the Decatur County CCommunity Foundation. The chart below lists the dates and times of the scheduled clinics:
