The packet includes information and forms for Healthcare Representatives, Living Will Declaration, Life-Prolonging Procedures Declaration, Out of Hospital “Do Not Resuscitate,” Organ and Tissue Donation, Healthcare Treatment Directive, Psychiatric Advance Directive and Power of Attorney.
Birth Certificates – Certified copies
Many entities require the presentation of a certified copy of birth certificates which are the official records of birth. These are not maintained by the Hospitals of birth. Certified copies of birth and death certificates are available from the Indiana State Department of Health (ISDH) or available from the local health department in the county where the event occurred. All requests require proper identification.
