Patient Portal

You can access your Patient Portal using the web-based and/or desktop version —  or access your health information with one tap using the app:

Visit the Apple App Store or Google Play to download the FREE app today! Search for “Meditech”, select one of the app options above, or click the buttons below.

You can now self-schedule appointments in your Patient Portal!

1. Visit Patient Portal and/or open your mobile app
2. Login
3. Select “Appointments”
4. Select “Schedule new”
5. Determine the appointment type you’d like from the available types (Family Medicine visit, Immediate Care visit, or a Mammogram Screening) and select “Schedule” under that visit type
6. Answer any on-screen questions, following on-screen instructions and guidance
7. Select “Next”
8. Select the appointment listed as “Select earliest available appointment”
   • • Type in the reason for your visit
     • Include any relevant comments
     • Indicate how the DCMH team should contact you with any questions
     • Select “Next”
     • Review your appointment details and when you’ve verified they’re accurate
     • Select “Submit”
9. OR if the earliest available appointment does not work for your schedule, select “View full schedule” for more options
   • • Select a date and time that better fit your schedule
     • Select “Next”
     • Type in the reason for your visit
     • Include any relevant comments
     • Indicate how the DCMH team should contact you with any questions
     • Select “Next”
     • Review your appointment details
     • When you’ve verified they’re accurate, select “Submit”

Access for Minors, Newborn to Age 13: You must complete the “Authorization for Proxy Access to Patient Portal” form

1. Download and print the form below
2. Complete the form
3. Submit your completed form to regrequest@dcmh.net or give your completed form to any Registration team member across DCMH’s locations
4. Once DCMH grants your access, the proxy will then be able to easily view their loved ones’ records within their own portal

Access for Minors, Age 14 to 17: You must complete the “Minor Patient Portal Consent” form

1. Download and print the form below
2. Complete the form
3. Submit your completed form to regrequest@dcmh.net or give your completed form to any Registration team member across DCMH’s locations
4. Once DCMH grants your access, the proxy will then be able to easily view their loved ones’ records within their own portal

Access for Adults, Age 18+: You must complete the “Authorization for Proxy Access to Patient Portal” form
