Clinical Social Work
The principle purpose of the Social Workers at Decatur County Memorial Hospital in Greensburg, Indiana is to provide services to patients and others who have social, psychological, environmental, and/or economical needs related to the impact of admission, diagnosis, treatment, discharge and/or aftercare.
Our social workers are available to inpatients, observation, outpatients, swing bed, cancer care, emergency, and home health care patients. Age of population varies from neonates through geriatric. DCMH patients involved with these services may request the assistance of one of our social workers.
Decatur County Memorial Hospital is staffed with master’s level licensed social workers. Should you need assistance or have questions, feel free to contact a social worker at 812-663-1176.
The social work department maintains the following list as a community service, providing valuable resources for those seeking assistance or direction.
Our social workers are available to provide, but not limited to, the following services: psychosocial assessment, discharge/transition planning, consultation, crisis intervention, education, community liaison, information and referral, and advocacy.
Registered Nurse/Care Manager
A care manager is a registered nurse who will typically make bedside visits with your physician and is available to answer questions about your physician visit. The care manager closely works with our patient’s physician and hospital team to coordinate care and prepare for the patients return home.
This includes IV antibiotics, ostomy, wound care supplies, and education regarding diagnosis, treatment and discharge plans.
Within a few days of going home, a registered nurse will call our patients to check on their health and answer any questions.
