Decatur County Memorial Hospital would like to congratulate Dr. Naveed Anjum Fazlani of Decatur County Primary Care for being awarded recognition by the National Committee for Quality Assurance in Diabetes.

NCQA’s Diabetes Recognition Program recognizes clinicians who have met standards demonstrating delivery of high-quality care to patients with diabetes. The NCQA Diabetes Recognition Program is a voluntary effort to identify clinicians who provide diabetes care consistent with the comprehensive diabetes care measures within the Healthcare Effectiveness Data and Information Set (HEDIS®)

Clinicians who achieve DRP Recognition show their peers, patients and others in the Diabetes community that they are part of an elite group that is publicly recognized for its skill in providing the highest-level diabetes care.

In the U.S., there are 5,967 providers and 1,981 facilities recognized with the DRP. In Indiana alone, there are only 133 providers and 41 facilities who are a part of this program, including Decatur County Primary Care and Dr. Fazlani.

O’Kane, President, National Committee for Quality Assurance. “By earning recognition, Naveed Anjum Fazlani, MD has demonstrated that he provides effective, evidence-based care to his patients with diabetes.”

Dr. Fazlani added, “This recognition, given to a very select group across the country, is based on improvement of diabetes care over a year. This is a testament to the patients in our community for their desire to improve and manage their diabetes. My patients have given me everything that I have asked for. This recognition is a reflection of this.”
