Free health screenings are made possible thanks to the Hospital Foundation of Decatur County and its generous donors.

After You Submit the Registration Request Form:

• • Only 25 spots are available per timeslot
  • You’ll receive a confirmation email within 3–4 business days if your preferred time is available
  • If your preferred time is full, Registration will call you to reschedule
  • You must receive a confirmation email with additional information to be officially registered
  • If you do not receive a confirmation email in 3-4 business days, please contact Registration at regrequest@dcmh.net
  • Requests submitted after hours, on weekends, or holidays will not be addressed until the next business day
  • Note: Requests are processed in the order received, and response times may vary depending on volume
