Sports physicals are for student-athletes with no pre-existing health conditions. If you are currently seeing a physician or taking prescription medications for an acute illness, please have your physical completed by your primary care physician. Visit DCMH Tree City Medical Partners on June 6 from 6-8 p.m. Physicals are first come, first serve and are only $20 cash or check. A parent or guardian MUST be present. DCMH has partnered with the Decatur County Department of Health to provide immunizations to any student needing them during the sports physical night. Please fill out the Pre-Participation Physical Evaluation Form (PPE) found here, and bring it with you.
