For more than 100 years, Decatur County Memorial Hospital has been committed to caring for our community. As healthcare continues to evolve, one thing remains essential: listening to the people we serve.
To strengthen that partnership, we are inviting our community to join our Patient & Family Advisory Council (PFAC).
What is the PFAC?
The PFAC is a group of community members, patients, and family caregivers who meet regularly with DCMH leaders to share honest feedback, perspective, and ideas. These conversations help us improve the patient experience and guide future decisions that impact our hospital and community.
The council is advisory, not decision-making — but your input directly informs DCMH strategies, goals, and initiatives.
Participation in the PFAC does not create an employment or contractual relationship with DCMH, and PFAC members are not authorized to make decisions on behalf of the hospital
The Role of a PFAC Member
Members participate in discussions about:

Meeting Details - What to Expect
You can expect the meetings to be:

Every other month at DCMH

Approximately 1-hour

Relaxed, conversational format

Food and beverages provided

Facilitated by the DCMH Marketing & Communications Manager

We aim to create a comfortable environment where honest feedback is encouraged and valued.

What You Can Expect From Us
After you submit the form, we will follow up by email or phone to set up an introductory conversation.
Please note that requests submitted after hours, on weekends, during DCMH event weeks, or on holidays cannot be reviewed until the next business day.
